COURSE REGISTRATION FORM

(For INDIVIDUAL PARTICIPANTS)

To register more than one person for the same course, click on 'Multiple Participants' and you can enter them all on one form. If you use the multiple participant form, every participant must be for the same course type and date.

2) Please make sure you are selecting the right Course Type, Date & Location. LMac Community CPR is NOT responsible if you select the wrong option nor liable for any costs those errors may incur.

4) In keeping with most current workplace policies, the LMac training room is a scent free environment so please refrain from using perfumes, colognes, creams or any thing else that could aggravate a scent sensitivity.
